MDVitals collects and logs Internet Protocol (“IP”) addresses. This log data is used in aggregate form to analyze website usage. Your data will not be sold, shared, or made available to any third parties.

Requirements for a Verifiable Consumer Request
• Provide sufficient information to verify your identity or authority to make the request.
• Clearly describe your request with enough detail to enable us to understand, evaluate, and respond.
We cannot respond to your request or provide your personal information if we cannot verify your identity or authority. Making a verifiable consumer request does not require you to create an account with us. Personal information collected during the request process will only be used to verify your identity or authority to make the request.
Response Timeline and Format
We strive to respond to verifiable consumer requests within 90 days of receipt. Our response will cover the 12-month period preceding the receipt of the request. If we cannot comply with your request, we will provide an explanation.
Non-Discrimination Policy
We will not discriminate against you for exercising your CCPA rights. This means we will not:
